What to bring with you:
State birth certificate (live birth record)
Up-to-date shot records
If you have not yet obtained a state birth certificate, the form to order one will be available at the enrollment.

Please get the most recent shot records from your child's doctor.
New this year:
Your child's doctor is now responsible for entrance screenings. These should be done during the "well-baby" visit. No screenings will be given at school sites prior to entry.
